STARFLEET MEDICAL
Starfleet Medical was created as an independent branch of the Starfleet on November 14, 2143. In the fall of 2146, the Office of Starfleet Medical was significantly expanded with the appointment of the first Starfleet Surgeon General. The headquarters of Starfleet Medical is located at the Starfleet Medical Center.

STARFLEET MEDICAL CENTER
The Starfleet Medical Center is one of three major headquarter facilities authorized in May 2138 per Starfleet Operations Directive #7. Groundbreaking for the new Starfleet Medical Center, originally scheduled for the spring of 2141, was delayed due to construction shortages as well as the small size of Starfleet Medical precluding the need for a large headquarters building. In September 2145 groundbreaking occurred for both a Starfleet medical headquarters as well as an attached hospital. Construction was completed in October 2146, but the facility did not immediately open due to a general shortage of doctors as well as a lack of Starfleet nurses (the Nursing officer designator having only been created a few weeks prior).
On December 18th, 2146, Starfleet formally dedicated the Starfleet Medical Center north of the grounds of Starfleet Headquarters. The facility was designated the headquarters of Starfleet Medical and the location of the Office of the Starfleet Surgeon General.

FIRST STARFLEET HOSPITAL

The First Starfleet Hospital is a major medical facility directly attached to the Starfleet Medical Center and considered a "tenant command". The 1st Hospital maintains its own chain of command, duty watches, and medical personnel; however, since the facility is considered a permanent annex of the Starfleet Medical Center, all non-medical needs such as security and maintenance are overseen by medical center duty personnel. The Hospital Director of the First Starfleet Hospital is also a direct subordinate to the Director of the Starfleet Medical Center.
Upon the opening of the Starfleet Medical Center in December 2146, the First Starfleet Hospital remained closed due to a shortage of trained medical personnel as well as a lack of regulations and directives concerning hospital operations. In January 2147, the Starfleet Medical Manual was published, with the opening of the First Starfleet Hospital scheduled for March of that year.
STARFLEET MEDICAL HISTORY
Starfleet Medical was first created in the late 2130s as a small internal office of Starfleet Operations known as the Medical Affairs Division. The first Starfleet medical facility was the Starfleet Headquarters clinic which offered routine medical care to headquarters personnel. A larger twenty-four-hour facility was then constructed at the Starfleet Training Center and provided medical care to Starfleet personnel as well as pre-induction physicals for Starfleet applicants. Serious medical emergencies or long-term care was outsourced to civilian hospitals in the San Francisco area.
The first Starfleet Surgeon General, Dr. Hindman Marazano who had previously served as Director of the Warsaw Medical University, was appointed in November 2146; prior to this time Starfleet Medical had been headed by a Medical Director. Due to Dr. Marazano's appointment directly to the flag rank of Commodore, his entry into Starfleet required United Earth Congress consent.
At the time of the appointment, the Starfleet Surgeon General possessed no staff and was simply the direct superior to the Director of Starfleet Medical. This was considered a temporary state of affairs and the following month, upon the opening of the Starfleet Medical Center, Dr. Marazano was promoted to Rear Admiral, with the former Office of Starfleet Medical becoming the new Office of the Starfleet Surgeon General.
